This medicine form went home with students on 9/25. Please complete the form and return it ASAP, but no later than 10/8/2025 at the mandatory parent meeting. If your child takes prescription meds, the form MUST be signed by a physician before it can be returned. DO NOT DELAY! Put in a call to your Dr. today to have them sign the form!
